INFORMATION FOR DR. NISHA SONI'S PATIENTS
Dr. Nisha Soni will be leaving Walkley Medical Centre and transferring her practice to Dr. Alura Riley on October 1, 2022.
Patients of Dr. Soni will automatically be transferred to Dr.Riley's roster for 6 months by the Ministry of Health and Long-Term Care. If you would like to remain a patient of Dr. Riley's, you will need to sign an enrolment form by March 1, 2023. This form can be obtained from our office. If you prefer to find your own family doctor, you can contact Health Care Connect for assistance at 1-800-445-1822 or online.
Your electronic chart will remain at Walkley Medical Centre with Dr. Riley. In order to have your chart transferred to your new family doctor, a signed authorization form must be sent to our office. The fees for chart transfers are not covered by OHIP and will be based on the time required to complete the transfer and the Ontario Medical Association's recommended rates.

Dr. Ardavan Assemi

Dr.Assemi is a proud graduate of Dalhousie Univeristy’s class of 1998. He went on to complete his degree in Family Medicine at McGill University and his residency at St.Mary’s Hospital in Montreal, Quebec. From 2000-2003 he provided long term locum coverage in Kemptville, Ontario, where he worked in the emergency room, provided inpatient care as well as in office family medicine. In 2003, in partnership with Dr. Naveen Soni, he opened a practice in Ottawa where he continues to provide comprehensive family medicine and clinical care to his patients.
Dr. Sumitha Ramachandran

Dr. Ramachandran has been in practice since 2004. She is a medical graduate of University of Calgary (2002), and did her family medicine residency in Ottawa. She is an associate professor with the University of Ottawa Department of Family Medicine and practiced inpatient medicine at The Ottawa Hospital General Campus for 15 years. Dr. Ramachandran also has expertise in treatment of varicose vein disease and is medical director of Capital Vein Clinic (www.veinclinicottawa.com).
Dr. Madelaine Kukko
Dr. Kukko has been practicing Family Medicine since 2022. She completed medical school and residency at The University of Toronto. She then returned home to Ottawa to complete additional training in Women’s Health and Low-Risk Obstetrics at The University of Ottawa. She provides antenatal care to pregnant women in the office and delivers babies at The Ottawa Hospital General Campus. She holds a focused practice designation in Obstetrics and Gynecology. Dr. Kukko accepts referrals for obstetrical care from patients and physicians. She is not accepting new family medicine patients

Dr. Naveen Soni
Dr. Soni grew up in Saskatchewan and graduated from the University of Saskatchewan School of Medicine in 1998. He completed his family medicine residency at McGill University in Montreal in 2000. Following his training, he moved to Ottawa. He began practice in small town Ontario with emergency room and hospital coverage, along with primary care. He has since settled into full time primary care practice exclusively.
Dr. Alura Riley
Dr. Riley graduated from Queen's University School of Medicine in 2013 and completed her residency in family medicine at the University of Ottawa in 2015. She began practice in family medicine and palliative care in Vancouver and has continued to provide comprehensive care to patients and families since her
return to Ottawa in 2017.
