Hospitalization

If you are hospitalized, please let us know by calling our office. We encourage you to book an appointment with your doctor within 2 weeks of your hospital discharge to ensure a smooth transition from hospital to home. It is especially important that you bring your medications to this visit as they are often changed in hospital.
